Overview
A character who is Disgraced has already been Warned and did not heed their Warning, or did something incredibly egregious in the first place.
Those who are Disgraced are prohibited from using powers or possessing weapons in the presence of sect officials and may not feed within the sect's territory. If the Disgraced is caught breaking the rules (no using powers or possessing weapons around sect officials, no feeding within sect territory), they can show contrition by offering the a major boon (or debt of blood) and the offended party will be obligated to accept it. If a Disgraced character refuses to offer this contrition, they may then become Forsaken.
The Disgraced title encourages poor (IC) treatment of the character in the following forms: nobody owes anything to disgraced characters (so a Disgraced character cannot leverage outstanding boons, favors and debts) and those who publicly insult a Disgraced character gains the fleeting status trait Praised (from a sect member authorized to assign it).
How Do I Avoid It?
If someone is attempting to mark you as Disgraced and you don't want to be Disgraced, then you can dodge it by offering a Major Boon or its equivalent to the directly offended parties. In the Sabbat, the equivalent is a Debt of Blood. In the Anarchs, the equivalent is a favor of the same level as a Major Boon.
If you have an ally with the Faithful status, you can convince them to spend their Faithful status to have you pardoned.
A character with Authority (the Prince) can pardon an instance of Disgraced.
How Do I Get It?
If you become Warned and continue to misbehave in the same manner that got you Warned in the first place, then you will graduate to Disgraced unless you're willing to offer a minor boon or its equivalent (like a sworn debt).
Sabbat
If you already carry the Warned status and break its censure, you are rewarded with Disgraced.
Anarchs
If you already carry the Loser status and break its censure, then you are rewarded with Disgraced.
Once a month, the Constable can use their Warrant status to administer Disgraced should they break a local sect law and refuse the lesser punishment of Cherry or Loser.
Camarilla
From The Prince
Once a month, the Prince may use their Authority status to mark someone Disgraced.
From The Keeper
Once a month, the Keeper Of Elysium may use their Guardian status to mark someone Disgraced if they use disciplines in Elysium, cause physical injury in Elysium, succumb to Frenzy, goad someone else toward Frenzy, or bring weapons to Elysium. In some extenuating circumstances, someone may merely be Warned (e.g., was clearly goaded into frenzying, did not actually instigate).
From The Harpy
Once a month, the Harpy may use their Prominent status to force a character to choose between physically leaving a particular scene or taking the Disgraced status. The Harpy will do this if they find the character dishonorable or discourteous and will warn the character to stop before escalating towards doing so.
Once a month, the Harpy may use their Guardian status to administer Disgraced to a character they find to be breaking a boon. In contrast to punishing someone they find generally dishonorable, the Harpy doesn't owe a warning to those who she can prove to have broken a boon.
